All plans are effective for one year from the day the premium is paid. Additional fees may be applied for certain treatments – such as gold crowns – but account holders will be notified before hand by our dental office. Scaling or periodontal deep cleaning are included in Routine Dentistry. All co-pays are due at the time of service. Financing of any kind is not available.
